dual credit/enrollment process & forms:
The dual credit process begins with a required meeting on the first Friday of the semester for all students in a FHS dual credit class, during their class. At that time, forms are given to students with deadlines for parent signature within 4 business days. By the due date of Thursday, January 11, 2018, by 3 pm, the student must have all completed paperwork (with CURSIVE signatures) & permission form IF it is required, together with the FHS transcript provided by the dual credit teacher at the meeting and, if applicable, the orange Scholarship Tuition form with required attachments to Ms. Andrea Richardson in the FHS Counseling Center. Failure to meet deadlines will result in no college enrollment. CHECK the guideline below the following links to see if the Permission Form is required to be attached to an individual application.
**CTE dual credit forms will be given to students by the teacher; student must check to see if a MAC Recommendation Form is also required (see criteria at the bottom of this page).
FORMS are located outside of Ms. Andrea Richardson's office door (back hallway, around the corner from the counseling center).
NOTE: If you need to drop a college class after enrolling for college credit, YOU must fill out the drop form for the specific college/university prior to the drop deadline listed in your paperwork copies or the institution's website; see Ms. Andrea Richardson. You may drop the college portion (but you still have to pay once you enroll; also not covered by any scholarship) and keep the high school portion, however if you drop the high school portion and forget to drop the college portion, then you will receive an "F" on your college transcript. DUAL credit means DUAL paperwork in DUAL systems completed by the STUDENT.

- 12th GPA 2.5-2.99 unweighted: FORM REQUIRED WITH Parent Permission Signature, Counselor Signature & Principal Signature (GPA 3.0+ unweighted is automatically qualified – recommendation form is not required)
- 11th GPA 2.5-2.99 unweighted: FORM REQUIRED WITH Parent Permission Signature, Counselor Signature & Principal Signature (GPA 3.0+ unweighted is automatically qualified – this form is not required)
- 10th GPA 3.0-4.0 unweighted: FORM REQUIRED WITH Parent Permission Signature, Counselor Signature & Principal Signature
- 9th Score a minimum of 90th percentile on ACT (28 composite or higher) or SAT AND GPA 3.0-4.0 unweighted: FORM REQUIRED WITH Parent Permission Signature, Counselor Signature & Principal Signature
